That’s good news: Patients who receive detailed instructions on how to care for 30% less likely to be returned to the hospital or visit the emergency department than uninformed patients.

That’s bad news: patients can not always get the information necessary for safe and successful discharge from hospital. The Commonwealth Fund reported that in the best condition, 91% of patients with heart failure were discharged home with written instructions, and only 51% in the worst states.

Here’s the key: discharge from hospital should be professionally planned and organized for you to have success. If not, then you are at risk for injury, misunderstanding, medical error, or the way back to the hospital.

Here’s what to do: insist on full implementation of a set of instructions. This should include detailed information about medication, aftercare, and what to do if your state change. You should also know who to turn to questions about his condition or treatment, as soon as you would at home.

Be sure to fully understand all the information you receive. Recent studies have shown that one half of adults have trouble understanding their requirements or discharge instructions. In addition, less than half the patients know their diagnosis, treatment plan and side effects of prescribed medication.

Patient Education experts agree that all medical information should be presented in such a way that is sensitive to their language, culture, as well as their level of understanding of the terminology of health care.
